Financial Aid Forms

For the 2024-2025 process, students who are chosen for verification will need to login to StudentForms to complete all necessary documents.

IF REQUESTED, FORMS LISTED BELOW MUST BE SUBMITTED TO THE FINANCIAL AID OFFICE. Our office images all documents and routinely destroys all paper.

All documents submitted to the Financial Aid Office must be signed. Unsigned documents will delay the processing of your financial aid.

*Please allow a 24-hour turn-around period for confirmation of receipt. During peak times, please allow a 48-hour turn-around period.

Need a copy of your Tax Return Transcript to complete your financial aid file? You have two easy and convenient options for getting copies of your federal tax return information (tax return transcript). Instructions are available on the IRS website.

Tax Transcripts must be mailed to your home address not to Clayton State University.

Parent PLUS Loan (For Dependent Students Only)

A William D. Ford Federal Direct Parent PLUS loan is a loan borrowed in the parent’s name for educational purposes for a dependent student enrolled at least half-time (6 credit hours) in a degree-seeking program and maintaining Satisfactory Academic Progress. While the parent borrower does not have to prove financial need, a credit check is required.

Graduate PLUS Loan

The William D. Ford Federal Graduate PLUS Loan is available to graduate students needing additional funding for educational purposes who are enrolled at least half-time (5 credit hours) in a degree-seeking program and maintaining Satisfactory Academic Progress. While the student borrower does not have to prove financial need, a credit check is required.

Loan Revision Form

All loan changes requested will be made using the Loan Revision form. Please complete this form if you would like to cancel your loans, adjust your loans to a different semester, change the amount of loans accepted or reinstate a declined or cancelled loan.
