Financial Aid Forms
For the 20223-2024 process, students who are chosen for verification will need to obtain their Required Action Letter from the DUCK Message Portal. All instructions must be followed on all Required Action Letters which may include completing, signing and returning the letter and submitting additional documentation to our office for processing.
IF REQUESTED, FORMS LISTED BELOW MUST BE SUBMITTED TO THE FINANCIAL AID OFFICE. Our office images all documents and routinely destroys all paper.
All documents submitted to the Financial Aid Office must be signed. Unsigned documents will delay the processing of your financial aid.
*Please allow a 24-hour turn-around period for confirmation of receipt. During peak times, please allow a 48-hour turn-around period.
